Call for Nominations
Dean L. David and Janet C. Hiner Distinguished Pharmacy Alumni Award
Award Criteria:
- Be a graduate of the University of Utah College of Pharmacy
- Be present to receive the award at the White Coat Ceremony
- Demonstrate continued commitment to the College of Pharmacy (examples include: teaching, precepting, volunteering, philanthropy, or other)
- Contribute meaningfully to the pharmacy profession (examples include: clinical, business, academic, research, industry, administrator, or other)
- Positively reflect the aims and the academic mission of the College of Pharmacy
To nominate an individual please forward a letter of recommendation describing how the individual fulfills the above criteria.
On a separate piece of paper, please also provide the name, title, and contact information, of yourself and the nominee. A brief statement describing your relationship to the nominee is also required.
Nominations are due by July 31, 2008.
Award presentation will occur at the Annual College of Pharmacy White Coat Ceremony held in September 2008.
